After your spot is confirmed, Tracy will reach out to discuss your care options, schedule your first 60–90-minute consultation, and learn what matters most to you.
Joining the waitlist does not collect payment. You do not need to select membership or fee-for-service yet. Tracy will discuss both options with you after your spot is confirmed.
CharmEHR patient portal access: You will not be able to access CharmEHR until your membership is confirmed and you have been created as a patient in CharmEHR. This should occur within 24 hours of membership confirmation.
Prepare for Your First Visit
Download and fill this out at your own pace before your first visit. After your care is confirmed and your CharmEHR portal is set up, upload it so Tracy can review your health history ahead of your consultation.
